Description
Overview of the medicine
Exipain 100 mg Tablet contains Aceclofenac, a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and inflammation associated with various musculoskeletal conditions.
Uses & Indications
Dosage
Adults
100 mg twice daily, preferably with or after food.
Elderly
No dosage adjustment is generally required, but caution should be exercised. Start with the lowest effective dose.
Renal_impairment
In mild to moderate renal impairment, no dose adjustment is needed. Contraindicated in severe renal impairment.
How to Take
Administer orally. Take the tablet whole with a glass of water, preferably with or after food to minimize gastrointestinal discomfort. Do not chew or crush.
Mechanism of Action
Aceclofenac acts primarily by inhibiting prostaglandin synthesis, a process mediated by cyclooxygenase (COX) enzymes. It inhibits both COX-1 and COX-2, leading to reduced production of inflammatory mediators.
Pharmacokinetics
Onset
Analgesic effect usually observed within 1 hour.
Excretion
Approximately two-thirds of the administered dose is excreted in the urine, mainly as hydroxylated metabolites, and one-third in the faeces.
Half life
Plasma elimination half-life is approximately 4-5 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached in 1.25 to 3 hours. Bioavailability is approximately 98%.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, to 4'-hydroxyaceclofenac and diclofenac, which are then conjugated.
Side Effects
Contraindications
- •Hypersensitivity to Aceclofenac, aspirin, or other NSAIDs
- •Active gastrointestinal bleeding or peptic ulcer disease
- •Severe renal or hepatic impairment
- •Severe heart failure
- •Third trimester of pregnancy
Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Reduced renal clearance of methotrexate, leading to increased toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Diuretics and ACE inhibitors
May reduce the antihypertensive effect and increase risk of renal impairment.
Warfarin and other anticoagulants
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include headache, nausea, vomiting, epigastric pain, gastrointestinal bleeding, rarely diarrhoea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, fainting or convulsions. In cases of significant poisoning, acute renal failure and liver damage are possible. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal.
Pregnancy & Lactation
Avoid during the third trimester of pregnancy due to potential effects on fetal cardiovascular system. Use with caution in the first and second trimesters only if clearly needed. Avoid during lactation as NSAIDs can be excreted in breast milk.
