Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor (PPI) that decreases the amount of acid produced in the stomach. It is used to treat conditions like gastroesophageal reflux disease (GERD), ulcers, and to eradicate H. pylori.
Uses & Indications
Dosage
Adults
For GERD: 20 mg or 40 mg once daily for 4-8 weeks. For H. pylori eradication: 20 mg twice daily for 7-14 days in combination with antibiotics. For Zollinger-Ellison Syndrome: Initial dose of 40 mg twice daily, adjusted as needed.
Elderly
No specific dose adjustment is generally required, but caution is advised in elderly patients with hepatic impairment.
Renal_impairment
No dose adjustment is needed for patients with renal impairment.
How to Take
Take Exomi MUPS orally, preferably at least one hour before a meal. Swallow the tablet whole; do not chew, crush, or break it, as this may compromise the enteric coating.
Mechanism of Action
Esomeprazole is a specific inhibitor of the H+/K+-ATPase (proton pump) in the gastric parietal cell. By acting specifically on the proton pump, esomeprazole blocks the final step in acid production, thereby reducing gastric acidity.

Mainly excreted in urine (about 80%) and feces (about 20%).

Approximately 1-1.5 hours
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 64% after a single 40 mg dose, increasing to 89% with repeated once-daily dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (CYP2C19 and CYP3A4).
Side Effects
Contraindications
- •Known hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased digoxin levels due to increased gastric pH.
Warfarin
Increased INR and prothrombin time, leading to risk of bleeding.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased serum levels of methotrexate, particularly with high-dose therapy.
Nelfinavir, Atazanavir
Reduced plasma concentrations of these antiretroviral drugs, leading to decreased efficacy.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Experience with esomeprazole overdose is limited. Symptoms may include drowsiness, tachycardia, nausea, diarrhea, headache, and dry mouth. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Esomeprazole is excreted in human breast milk; therefore, caution should be exercised when administered to a nursing mother.
