Overview of the medicine
Livocard-Plus 75 mg Tablet is a combination medicine containing Clopidogrel and Aspirin, used to prevent blood clots in various cardiovascular conditions.
Uses & Indications
Dosage
Adults
Acute Coronary Syndrome (ACS): Loading dose of Clopidogrel 300 mg (or 600 mg for PCI) followed by 75 mg once daily, in combination with Aspirin 75-150 mg once daily. For established atherothrombotic disease (MI, stroke, PAD): 75 mg Clopidogrel once daily in combination with Aspirin 75 mg once daily.
Elderly
No specific dose adjustment is generally required for elderly patients.
Renal_impairment
Caution is advised in patients with severe renal impairment due to limited experience. No dose adjustment is generally required for mild to moderate impairment.
How to Take
Take the tablet orally once daily with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Clopidogrel selectively inhibits the binding of adenosine diphosphate (ADP) to its platelet receptor P2Y12, thereby inhibiting ADP-mediated platelet activation and aggregation. Aspirin irreversibly inhibits cyclooxygenase-1 (COX-1) enzyme, preventing the synthesis of thromboxane A2, a potent inducer of platelet aggregation.
Pharmacokinetics
Onset
Clopidogrel: 2 hours to 7 days for full effect. Aspirin: Within 1 hour for antiplatelet effect.
Excretion
Clopidogrel and its metabolites are excreted in urine (approx 50%) and feces (approx 48%). Aspirin and its metabolites are primarily excreted renally.
Half life
Clopidogrel (active metabolite): approx 8 hours. Aspirin (acetylsalicylic acid): 15-20 minutes; salicylic acid (metabolite): 2-4 hours.
Absorption
Clopidogrel is rapidly absorbed from the gastrointestinal tract, but undergoes extensive first-pass metabolism. Aspirin is rapidly and completely absorbed from the GI tract.
Metabolism
Clopidogrel is a prodrug, metabolized to its active thiol metabolite primarily by CYP2C19. Aspirin is rapidly hydrolyzed to salicylic acid by esterases in the GI mucosa, liver, and blood.
Side Effects
Contraindications
- •Hypersensitivity to Clopidogrel or Aspirin or any component of the product.
- •Active pathological bleeding (e.g., peptic ulcer, intracranial haemorrhage).
- •Severe hepatic impairment.
- •Third trimester of pregnancy (due to Aspirin).
- •Children and adolescents under 16 years of age with fever (due to Reye's syndrome risk with Aspirin).
Drug Interactions
SSRIs/SNRIs
Increased risk of bleeding.
Thrombolytics
Increased risk of bleeding.
NSAIDs (e.g., Ibuprofen, Naproxen)
Increased risk of gastrointestinal bleeding due to synergistic effect with Aspirin.
Oral Anticoagulants (e.g., Warfarin)
Increased risk of bleeding due to combined antithrombotic effects.
Proton Pump Inhibitors (PPIs) (e.g., Omeprazole, Esomeprazole)
Concomitant use with Clopidogrel may reduce its antiplatelet effect by inhibiting CYP2C19. Avoid concomitant use.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of Clopidogrel overdose may include prolonged bleeding time and subsequent bleeding complications. Aspirin overdose can cause salicylism (tinnitus, headache, confusion) and severe metabolic disturbances. Management involves supportive care, gastric decontamination (if recent ingestion), monitoring for bleeding, and correcting fluid/electrolyte imbalances. Platelet transfusion may be considered for severe bleeding.
Pregnancy & Lactation
Category D for Aspirin in the third trimester (risk of premature closure of ductus arteriosus). Avoid in the third trimester. Use with caution in other trimesters if benefits outweigh risks. Clopidogrel is Category B. Both Aspirin and Clopidogrel metabolites are excreted in breast milk; therefore, use with caution or consider alternatives during lactation.
