Description
Overview of the medicine
Exipain is a combination medicine used for the relief of mild to moderate pain and fever. It contains paracetamol, an analgesic and antipyretic, and caffeine, a mild stimulant that enhances the pain-relieving effects of paracetamol.
Uses & Indications
Dosage
Adults
1-2 tablets every 4-6 hours as needed. Do not exceed 8 tablets in 24 hours.
Elderly
No specific dose adjustment required, but use with caution in frail elderly patients.
Renal_impairment
Use with caution. Consider extending dosing intervals in severe renal impairment.
How to Take
Exipain tablets should be swallowed whole with water. Can be taken with or without food.
Mechanism of Action
Paracetamol works by inhibiting prostaglandin synthesis, primarily in the central nervous system, thereby reducing pain and fever. Caffeine acts as a central nervous system stimulant, constricting cerebral blood vessels, which may contribute to headache relief, and also enhances the analgesic effect of paracetamol.

Metabolism
Paracetamol is primarily metabolized in the liver. Caffeine is extensively metabolized in the liver by demethylation and oxidation.

Drug Interactions
Alcohol
Increased risk of liver damage with chronic alcohol consumption and paracetamol.
Warfarin
May increase anticoagulant effect with prolonged use of high dose paracetamol.
Cimetidine
May inhibit caffeine metabolism.
Other paracetamol-containing products
Increased risk of overdose.
Certain antibiotics (e.g., Ciprofloxacin)
May reduce caffeine metabolism, leading to increased caffeine levels.

Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of paracetamol overdose include nausea, vomiting, abdominal pain, and liver damage. Caffeine overdose can cause nervousness, insomnia, anxiety, and heart palpitations. In case of overdose, seek immediate medical attention. Treatment may include gastric lavage, activated charcoal, and N-acetylcysteine for paracetamol toxicity.
