Description
Overview of the medicine
Livocard Plus is a combination medication containing Atorvastatin, a statin, and Olmesartan Medoxomil, an angiotensin II receptor blocker. It is used to treat high blood pressure (hypertension) and high cholesterol (dyslipidemia) when both conditions occur together.
Uses & Indications
Dosage
Adults
Usually one tablet once daily. Dosage may be adjusted based on individual response to components.
Elderly
No specific dosage adjustment required, but caution is advised. Initiate at lower end of dosing range.
Renal_impairment
Caution in severe renal impairment (CrCl < 20 mL/min for Olmesartan). Monitor renal function.
How to Take
Oral. Can be taken with or without food.
Mechanism of Action
Atorvastatin reduces cholesterol synthesis by inhibiting HMG-CoA reductase. Olmesartan blocks the angiotensin II type 1 (AT1) receptor, preventing vasoconstriction and aldosterone release, thereby lowering blood pressure. The combined action helps manage both dyslipidemia and hypertension.

Onset
Atorvastatin: Lipid-lowering effects typically seen within 2 weeks. Olmesartan: Blood pressure reduction starts within 2 weeks, maximal effect by 8 weeks.
Excretion
Atorvastatin: Primarily biliary/fecal. Olmesartan: Mainly renal (50-65%) and biliary/fecal (35-50%).
Half life
Atorvastatin: About 14 hours. Olmesartan: About 13 hours.
Absorption
Atorvastatin: Rapidly absorbed, Cmax within 1-2 hours. Bioavailability approx. 12%. Olmesartan: Rapidly absorbed, Cmax within 1-2 hours. Bioavailability approx. 26%.
Metabolism
Atorvastatin: Extensively metabolized by CYP3A4 to active ortho- and parahydroxylated metabolites. Olmesartan: Not metabolized by cytochrome P450 enzymes; minimal systemic metabolism.

Contraindications

- •Active liver disease or unexplained persistent elevations of serum transaminases

Drug Interactions
NSAIDs
May reduce antihypertensive effect of Olmesartan and impair renal function.
Ciclosporin
Increased Atorvastatin exposure.
Grapefruit juice
May increase Atorvastatin levels.
Gemfibrozil/Other fibrates
Increased risk of myopathy with statins.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia with Olmesartan.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Management is symptomatic and supportive. Atorvastatin: No specific antidote. Olmesartan: Hypotension is the most likely symptom. Treat with IV fluids, monitor vital signs.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation due to potential fetal harm (Olmesartan) and unknown risks (Atorvastatin). Women of childbearing potential should use effective contraception.
